2019 Community Champion Nomination

We are now accepting nominations for 3rd Annual Wellness Now Community Champion Award. Community Champions will be selected for their service and dedication to the community by using their lived experience to give back. Community Champions can come from all different back grounds and experiences but have one thing in common – they spend time connecting with and changing their community for the better, whether they are paid for it or not. Community Champions work toward a healthier community and typically live in the community they serve.

Examples of past winners include those living in long term recovery from drugs and alcohol who are now Peer Recovery Support Specialists, a retired teacher who canvasses for pro-education political candidates, and a former teen mother who formed a non-profit to educate adolescents on safe sex and reproductive health.

Nomination forms are due by Monday, November 11th.

For questions, please contact Yanet_trejo@occhd.org or call (405) 419-4016.

The selected Community Champions will be recognized at the December 11th Wellness Now Coalition meeting at the Northeast Regional Health and Wellness Campus from 12-1:30 pm. Those selected will receive recognition from their state representative and will get to speak for 10 minutes at the Coalition meeting to share their story.
